(Golf 4 Wellness) Tournament Golfer/Sponsorship Package



This year we will endeavour to raise 65,000 to purchase a much needed 15 passenger wheelchair
accessible van for the adult day support program which will be utilized daily within the Okotoks, Black Diamond
(Oilfield) and High River communities. In addition a "portable overhead tracking
lifter" system is required to move mobility impaired individuals safely & comfortably within their homes.
This would be available through our equipment loaner pool.

The Oilfields/Okotoks Health Foundation’s Mission Statement is to “raise funds to enhance and
support community health services in areas not provided for by government funding”.

The 2006 “GOLF 4 WELLNESS” Tournament will be hosted at the D’Arcy Ranch Golf Club - Okotoks

We are all excited and committed to ensuring that this tournament will be a day of fun, laughter
and rejuvenation. With community support, last year we were able to support the purchase of the Telehealth Care
System for the Okotoks Health & Wellness Centre. The Golf 4 Wellness Tournament is our signature event for fundraising.

A DAY OF PLAY & WELLNESS
Registration will commence at 12 noon prior to the 1:15 p.m. shotgun start. Following an exciting
18 holes, we will host a cocktail reception and spectacular banquet. The evening entertainment will be
fast paced with a few unique surprises.

OH MY WORD!





I seem to be sporting a spare tire at my mid section! Help support me in my challenge to rid
myself of these unwanted lbs.
The Okotoks Lions Club are sponsoring "Community Countdown",... a challenge to lose 5,000. lbs. through
individual pledges with the proceeds to support the purchase of a 15 passenger wheelchair accessible van for community
residents through the Oilfields/Okotoks Health Foundation (OOHF). Residents who are confined to wheelchairs
rely on assistance/transportation to get mobile and interact with others. Sometimes this is their only outing and
social interaction.
My goal is to shed 10 lbs. between now and June 9th. If you would like to support this worthy cause (van),
please email me back with a "pledge/lb." or fixed amount. Tax receipts provided for donations of $25. or more.
